2019 Nomination Forms

This year, all nomination forms are available in MS Word below. Click on the blue content in the category that you wish to nominate. The documents can be printed, filled out and mailed, or you may submit nominations by email.  

We do ask that you fill the forms out in their entirety to ensure consideration for the award.  Criteria to be aware of includes:

                    1. Nominee's direct contributions supporting the organization,

                    2. Specifics on achievements,

                    3. What types of programs have they launched,

                    4. How does the nominee motivate others, and

                    5. Write the nomination as if it were a blind submission.

Please do NOT refer to the person's name other than the general information about the nominee in the nomination form.

This will make it easier for our judges to vote. Thank you!

If you prefer to submit your nominations by e-mail you may download a word document below, and mail it to the address on the forms. Documents that are missing information may be rejected and must be resubmitted prior to the deadline of Friday, June 28th, 2019. 

This award is presented to an individual or family with a proven record of exceptional generosity; who, through direct financial support, has demonstrated outstanding civic and charitable responsibility; and whose generosity encourages others to take leadership roles in philanthropy and community affairs.

This award is presented to an individual who has demonstrated exceptional skills in coordinating and motivating groups of volunteers for one or more major fundraising project(s) for the benefit of a charitable institution, as well as a commitment to the advancement of philanthropy.

This award is presented to a business, corporation or foundation that has demonstrated outstanding commitment through financial support and encouragement and motivation of others to take leadership roles in philanthropy and community affairs.

Nominate your student/youth or student/youth group as Outstanding Youth in Philanthropy! The Greater Tri-State Chapter of the Association of Fundraising Professionals Outstanding Youth in Philanthropy award to recognize an individual or group who has demonstrated outstanding commitment to the community through direct financial support, development of charitable programs, and leadership in philanthropy. Two categories will be recognized this year. Be sure to choose on the form which category you are nominating for.

This award is designed for an outstanding individual fundraising professional who practices his/her profession in an exemplary manner.

JUDGES AWARD

The AFP Greater Tri-State Chapter presents this award to persons or organizations deserving of special recognition. 

All award recipient(s) will be honored during the National Philanthropy Day® Luncheon on Thursday, November 14, 2019.
